Rates
$135 per 45 minute session

Insurance
Dr. Gold is only a contracted provider for Arizona Foundation for Health Care Insurance.  If you are not covered by this insurance, please check to see if you have any "out-of-network benefits" for counseling.  If you do have this type of coverage, your counselings may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:

• Do I have mental health benefits?
• What is my deductible and has it been met?
• How many sessions per calendar year does my plan cover?
• How much does my plan cover for an out-of-network provider?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

Package Rates (available upon request)
Reduced fee services are available if you purchase a set of sessions in advance.  Please be aware that if you decide not to use all of the sessions you have purchased as part of the package, you will not receive any reimbursement for unused sessions.  

12 sessions (one time purchase of 12 @ $120 per session = $1,440           SAVINGS of  $180

20 sessions (one time purchase of 20 @ $110 per session = $2,200           SAVINGS of  $500


Payment
Visa, Mastercard, and Discover are accepted methods of payment.  Debit or Credit Cards only, no checks please.

Cancellation Policy
If you do not show up for your scheduled appointment, or you have not notified us that you are cancelling your appointment at least 24 hours in advance, you will be required to pay the full cost of the treatment as booked.
